To provide the best care to all of our patients, our healthcare providers would like your follow-up as requested for the optimal maintenance of any medical conditions.  Please note that constant monitoring of your health needs leads to better long term health benefits.  Kindly make sure you allocate time in advance for your follow-up appointments.

Medication Refills:
Please allow 48 hours for a refill of your existing medications.  Also, note that the provider may require you to have an office visit to renew your prescription.

Refill of Controlled Drugs/Narcotics:
Please note that the refill of controlled drugs is very limited and is preferable with a face-to-face appointment with our medical providers.

Required Patient information:
It is essential that we have up-to-date patient contact information.  As such we kindly request that you notify us as soon as possible if there are any changes to your contact details, such as your home address, home phone number, cell phone number, and email address.

Patients should have their photo ID available at time of appointment.

Cancellation Policy:
Cancellation or rescheduling policy requires that you cancel your appointment within 24 hours or you will be charged a $20 penalty fee.
