Co-payments and Deductibles

• As a service to our patients we bill your insurance company for all charges incurred at our Family Practice at time of visit.
• We expect payment of all fees, co-payments and deductibles at the time of your visit.
• We accept Cash, Visa, MasterCard, and Discover. Please present your insurance card at time of visit.
